A person could contribute more than one diagnostic category if they had admissions for different primary diagnoses, so the first at-risk period started at the first admission and was attributed to the primary diagnosis of this admission; after admission for a second primary diagnosis, the subsequent time was attributed to this second diagnosis and so on. SMRs in the paper were expressed as percentages rather than as ratios; ratios have been reported here.
